Default Transmission shifting problem

Hello everyone. My mom owns a 2000 Ford Taurus wagon with the base 3.0 engine and AXOD transmission. Here is the problems: sometimes when you start it up and take off from a dead stop the speedometer doesn't work and it won't shift out of 1st gear. Eventually once it revs up to about 4 to 5 thousand RPM's it will shift very hard into second and third while the speedometer needle floats between 0 and 30 MPH. It also shifts kind of hard probably 80% of the time from 1st to 2nd. Also the ABS light is on and the other day when it didn't want to shift once it shifted the transaxle light was flashing for 10 to 15 seconds. Any ideas? Sounds like the trans speed sensor.

Thank you, we replaced the input speed sensor about 6 months ago and it fixed the problem for a month or so then it acted back up. I'm thinking about replacing the output sensor but figured i'd get somebody's opinion first.
