I finally removed the upper intake manifold and pulled off the fuel injectors, 1 by 1 and cleaned them out. I also swapped #4 fuel injector with one of the other ones. Had to replace the intake gaskets, fixed a small vacuum leak, and replaced the temperature and coolant sensors and finally got it inspected and it's GTG (Good to go).
