96 E150 Running like the choke is stuck!

Default 96 E150 Running like the choke is stuck!
My 96 5.8 E150 isputting out dianostic codes of P0153,P0401,+P1151.This last code on the tester says Manufacturer Control Fuel Air Metering,This is not what I found when I looked up that code.It said lack of up stream heated oxygen sensor switch lean bank #2.Can any one help?

The P0153 and P1151 are for O2S21 (bank 2, sensor 1) Lack of switching. Could be a vacuum leak, misfire, exhaust leak or just a bum O2 sensor causing this.
