5.4 plug blow out

5.4 plug blow out
Hello the fire,
I have a 2002 f-250 superduty w/ 5.4L engine. It blew out the #6 plug yesterday. My question is: when the threads fail can the plug slip down into the head and be hit by the piston? My plug was smashed on the end.
