06 Freestar Wrench light-Limp mode

Hello, I purchased a an '06 Freestar 3.9L, back about a month ago. it had some jerking into gear via the transmission on occasion. Recently drove about 8 miles, got to my destination, restarted, and drove onto freeway, then all of a sudden the wrench light came on, I involuntarily decelerated and pretty much limped home. The vehicle will not go over 30 mph, and that's pushing it. I had the trans fluid-filter changed hoping it was a clogged filter issue but no dice, I've disconnected the negative terminal, then restarted, hooked up battery+ terminal connected to - terminal disconnected. I disconnected both battery terminals and hook them together for 30 minutes but I'm still having issues with "limp in mode". Does anyone have any suggestions? Thank you.

Are there any codes when you scan it?

If you don't have access to a scan tool or code reader places like Autozone will check your vehicle for codes usually free of charge. If you get any code(s) post them here and we might be able to offer some assistance.
