Default Spark Plug blow outs

At 100K I changed my spark plugs. At 115K I had on blow out. A new experience for me. I was able to fix it by using a kit sold at auto zone. I then backed each plug out, and re-tightened to specs with a torque wrench. I now have 147K and another plug is doing the same thing. I heard the tapping again, and I now know what it is. I tried to remove the plug, except the threads on the head are shot. I tried putting the socket on the plug and pulling up while trying to back it out. That did not work. I could just keep driving until it pops out. Does anyone have a better idea on how to remove it?

Also what could be causing multiple plugs to blow out?

Thanks!
