97 escort 2.0 SOHC timing belt issue

Hello,
I recently did a timing belt for a freind. 1997 escort with 2.0 SOHC. The belt was slightly off center on the cam pully only. everything else was ok. I didnt see any wear on it to indicate it had been that way for any length of time. The cam timing appeared to be off by 1 tooth and the tensioner still worked but seemed weak.
I bought a Kit and did the waterpump, tensioner and belt. New belt rides the same way. and its making a bit of noise on the belt cover. I cant tell why it is happening. can any one explain ???
