Default 99 E150 4.2 V6 EGR code

I'm getting the dreaded P0401 EGR insufficient flow code on my 99 E150 4.2. I've cleaned the EGR passages, replaced the DPFE and hoses, and the EGR valve. I'm STILL getting the code. What else could I have missed? The engine only has 8K miles on it.

Passages at the throttle body are clear/cleaned?

Chassis has 253K and the engine has 8K.
