Default Reese hitch 36050 won't fit

I found a used Reese hitch receiver, which I think was manufacured around 1995, that is supposed to fit my 1992 E150 . It didn't come with directions, and the only ones I've found online do not show the diagram that matches my configuration. I think the diagrams are generic for several manufacurers, such as Draw-Tite and Reese. My hitch has no "dip" in it to allow it to clear the bolts for the crossmember. It does have holes that seem to be for that, but that leaves only one bolt behind the crossmember and not two like the diagram shows. The hitch also only came with four bolts, when it certainly needs six.

Anyone have a Reese 36050 on an E150, 138" wheelbase, that can describe what the fastener configuration is under there?
