Default 2005 f150

I have a 2005 F150 with a 5.4 2WD with 118,000 Miles

If I am driving up a hill or some type of incline and I slowly accelerate my truck will skip//jerk on me. But if I gas it up a hill, it won’t do it at all. It’s been doing it for a few months and I’m just now doing something about it. Any idea what it might be?
