Default 1999 f-250 5.4
What is the hose that comes out the back side of the throttle body and goes into the valve cover ?

changed my plugs this week and that hose is damaged

PCV hose. While you are at it, change the PCV valve too if you haven't changed that for a while, it's only a few bucks.
Check rockauto and use 292739446259187330 at checkout.
