Hose assembly around thermostat

Default Hose assembly around thermostat

I have a 2001 Ford Taurus 24 valve DOHC. A rubber tube coming off of the coolant resorvoir tube that T-'s into one of the radiator hoses has cracked and is now leaking fluid. I went to Ford parts and turns out, it is one big replacement part with thermostat and housing included. Where the thermostat housing goes into one of the radiator hoses, there is a short piece of hose between. It almost looks like it has been heat shrunk on. Is there anyone who has any tips for replacing this whole piece? There are multiple hose connections on it and some are very hard to get to.

I have the same problem with the same part. I do not know how to get the old "elbow" Dealer parts term to ge the new one on.
