Default Engine Code Problems 97 Ford Escort

Having issues with a 97 Ford Escort Wagon 2.0L
1) Sometimes upon starting it will stall
2) Now randomly i'll be going 100 km/h and the car will go down to 20 - 40 km/h and have a funny sound. Sometimes a small backfire. This lasts for anywhere from 10 seconds to 2 minutes

3) When turning the car off the Radiator Fan turns on as well as what sounds like the fuel pump. This happens even when the car has been off for hours and gets turned on for 20 seconds. I have had to disconnect the fuse to stop the fan.
4) Attached my analyser and it popped 3 codes:
1- P0135 - Oxygen O2 Sensor Heater Circuit Malfunction (Bank 1, Sensor 1)
2 - P1132 - Lack of HO2S-11 Switch, Sensor Indicates Rich A HEGO sensor indicating rich at the end of a test is trying to correct for an over-lean condition. The test fails when the fuel control system no longer detects switching for a calibrated amount of time. See Possible Causes for DTC P1130
(Didn't pop this one, got this from a site) P1130 - Lack of HO2S-11 Switch, Fuel Trim at Limit The HEGO Sensor is monitored for switching. The test fails when the HO2S fails to switch due to circuit or fuel at or exceeding a calibrated limit.

Electrical:
Short to VPWR in harness or HO2S
Water in harness connector
Open/Shorted HO2S circuit
Corrosion or poor mating terminals and wiring
Damaged HO2S
Damaged PCM
Fuel System:
Excessive fuel pressure
Leaking/contaminated fuel injectors
Leaking fuel pressure regulator
Low fuel pressure or running out of fuel
Vapor recovery system
Induction System:
Air leaks after the MAF
Vacuum Leaks
PCV system
Improperly seated engine oil dipstick
EGR System:
Leaking gasket
Stuck EGR valve
Leaking diaphragm or EVR

Base Engine:
Oil overfill
Cam timing
Cylinder compression
Exhaust leaks before or near the HO2S(s)
3 - P1443 - EVAP emission control system control valve malfunction (This one has been there for awhile)

Now while checking out the car I discovered a crack about 4 centimeters long just before the second sensor just after the catalyst Converter you can see light through it so definitely a big leak. Would that be the most likely cause for the P1132?
What about the P0135? Does that refer to the front O2 sensor in the exhaust manifold?
And would any of these codes possibly be related to why my radiator fan and Fuel pump are turning on when the car is shut down?

Thanks,

Mike

Also forgot to mention. Disconnected the O2 sensor (the one in the exhaust manifold) and the fan turned off. Plugged it back in and a clicked started under the air filter for a few seconds and the fan turned on instantly. When unplugged the fan went off again...
