Default Bad tranny?

Recent repairs.
Water pump/serpentine belt about 2 months ago.
Fuel pump, filter, a couple weeks ago, this was due to my car having trouble starting after driving around, running from store to store and being hot, it basically worked, until about three days ago, then trouble starting again.
IAC was cleaned out (not my choice), last week, due to rumbling, almost stalling, when in park. I feel the issue got worse, one day started my car and after it settled down the car sound like, felt like, it wanted to stall, but was shaking really bad.

Car was sluggish, and rough today. Unusual.
Then a few hours later, loud clunk/jump when I went to accelerate, it sounded like, and felt like, something broke.

After this happened, the rest followed.

The car would jump into/out of gear, or shudders badly as gears were about to shift.
When braking the car shudders, or jumps forward.
One point, there was a rattling.
Running a bit rough.

After I got home, this is what happened:
Put the car in park and it surged forward about a foot.
Put it in reverse, to fix my parking, and it surged forward, but would not reverse while on the gas.
Turned the car off and tried again, car surged forward in reverse, and I was able to back it up.
Pulled it up, put it in park, surged forward again.
Put it in reverse, it surged forward. I tried to back up, it was jumping back.
Pulled up, and again, surged after putting it in park.
Tried it an hour later, again, started up fine, but surge when I put it in reverse, but backed up fine.
However, it would not drive forward, put it in park, surged.

Engine sounds fine, or normal. Fluids are good.

I'm truly convinced my tranny is done.
