Default 2011 e-250-5.4 "O2 sensor not complete"

I am failing Emissions because two tests are "not ready". The O2 sensor not ready and Heated O2 sensor not ready. I've driven about 250 miles city/hwy since test and it is still tests bad. No DTC's, CEL, or drivability issues. Any clue where to look for this one?

We may not all agree on this, but this is where I would start.
Confirm the oxygen sensor heater circuit is functioning.
Confirm the engine temp is reaching the operating temp
When the operating temp is reached, observe what the oxygen sensor is doing and see if it responds to my adding fuel and response when creating a large vacuum leak.
Before going any further would at least have an idea if those systems are working.

The engine gets to 190 no problem. I checked the fuse for the O2 heaters. BTW would not a bad O2 throw a code?

It can and does, but it doesn't always say the sensor is malfunctioning. It does throw codes that affect other functions and you need to know what those codes mean..

NO DTC's only the aforementioned not ready indications.
