If its anything like my 01, you insert the assembled rod, (3 rods just above your radiator) into the hole in the rear bumper just left and up of your liscence plate. Crank counter clockwise I believe and it slowly lowers the tire to the ground.

For Regular Cab,, thejack, jack handle and lug wrench are behind the interior trim panel/passenger side of the cab.. For Super Crew and Crew Cabs, the hardware/tools are under the rear seat/passenger side.. The spare lowering is as FordNutz described...
