Default E-250 Transmission problems

I have a 1995 E-250 van converted for handicap purposes the floor was lowered 6" it has about 111,000 miles. The transmission has started having a pretty hard bump or jerk when you take off from a dead stop, and now it feels like it's slipping on take off. I had a friend check it with a scanner all he said it gave him an error code for the Throttle Position Sensor, and some kind of heating or cooling something. Does anybody have any ideas or is the transmission probably going out?

Is the lite on the shift lever on or blinking? Usually this points to a trans malfunction, but not always. This is important to know. hanky
