Default Rear disc conversion

My van came to me with a rear disc conversion done by the previous owner. It’s a 1996 e-150 club wagon, the conversion did not include a parking brake setup so that has been non functional since I acquired the van. I am now in need of rotors and pads, only problem is I have no idea what I need to buy as I have no idea what components came with this kit or who the kit came from.

So where do I start?

There are surely other means to try, but you might consider taking the old parts to a good auto parts supply store and see if they can match up what you have.
