2014 f150 ecoboost blown piston with 42k miles. Engine was striped down only thing replaced was the piston everything looks good with the rest of the engine. Wondering if anyone else has had a problem or heard something?

 

What cylinder was this piston from?
What did the cylinder walls look like?
Were any of the pieces found in the oil pan or oil filter?
Was any check made to determine if oil cooling nozzle is/was restricted?

Wall looked brand new
Negative just one whole piece was broke off

Made a tick sound is how I figured it out..no engine lights or engine problems drove perfect

 

If it was checked I wasn't told nor did I even know to ask
